After booking, you'll receive your comprehensive questionnaire once your appointment is confirmed. Please complete it at least 48 hours before your scheduled appointment so your provider can review your responses and make the most of your evaluation session. The form takes about 30–40 minutes and can be saved and returned to if needed.
Ongoing ADHD Care — $175/month
Optional. After your evaluation, if ADHD treatment is right for you, our monthly membership keeps your care on track — without the runaround of booking one-off visits.
Your membership includes
- Quarterly follow-up visits (30 minutes each) with your provider
- Monthly medication refills, processed within 2 business days
- Secure messaging with a 1-business-day reply for non-urgent questions (business hours)
- An annual comprehensive review (60 minutes) to reassess your treatment plan
- Care coordination — brief notes to your therapist or primary care provider, if you choose
- Monthly symptom check-ins so we can catch changes early
- Lab-monitoring guidance (labs available at cost through our lab partner)
What's not included
- Crisis or emergency care
- Same-day, after-hours, or weekend response
- Therapy sessions
- Additional visits beyond the quarterly schedule (available at $175 each)
- Care for conditions outside ADHD (referred out, or ask about our concierge psychiatric care)
This membership is focused ADHD management — not comprehensive concierge psychiatric care. If your needs grow beyond ADHD, we'll talk about the right option for you. Medication, including any controlled substance, is prescribed only when clinically appropriate and per our standard visit schedule.
